The Hospice Care Team

It takes many different skills to meet the physical, emotional, and spiritual needs of hospice patients and those who love and care for them. The Community Healthcare of Texas hospice care program offers you the full resources of our entire team.

At Community Healthcare of Texas, we believe in a patient-centered approach to care, where the patient and their loved ones direct their care with a focus on their particular needs and wishes. Members of your care team will meet regularly to discuss your needs, share ideas and make plans that you will review. 

Your Hospice Care Team Includes

Other professionals may be called on to assist in the patient’s care from time to time. These additional services may include:

• Homemaker Services: Homemakers can provide light-duty housework in the patient area when deemed necessary by the Hospice team and patient/patient caregiver.
• Therapy Services: Available services include nutritional counseling, physical therapy, respiratory therapy, occupational therapy, ostomy care, and wound care. 
• Inpatient Care: A patient may qualify for inpatient care at our dedicated hospice house or one of our inpatient hospice wings to control acute pain and other symptoms or provide respite for the caregivers.
• Continuous Care: Our hospice team will provide crisis management in a patient’s home for short periods when necessary.
